- Feedback Form - A simple feedback form to use in classes. Feedback from completed forms is sent directly to your email address. Contact Cheri Smith if your name is not included on the form.
- Library Instruction Request Form - This is the form that faculty on campus can use to request library instruction sessions.
- Instruction Report Form (for ND Librarians) - Please note: You should file Instruction Reports in the Desk Tracker system, effective September 2010. Contact Sherri Jones (sherri.l.jones@nd.edu) if you need an account number and/or user name and password.
